To allow us to serve you better on your first appointment, be sure to bring:
- Identification
- Insurance cards or forms provided to you by your insurance
company - A list of any current medications you may be taking
- Any recent x-rays if they are available

Payments
Payments are due at the time of service. For your convenience, we are happy to accept Visa, MasterCard and American Express and will file all insurance on your behalf. Alternative payment methods are available upon inquiry.
Financing Options:
Insurance
We deal with many different insurance companies and will be happy to file a dental claim, where applicable, with your insurer on your behalf.
Cancellation
In case you may not be able to make your appointment, please provide us with 24 hours notice so we can schedule this appointment for another patient. If a patient doesn't not show up for an appointment without notice, a fee may be charged.
